Duties & Responsibilities
- Provide comprehensive primary care services to a panel of senior patients
- Adopt a value-based approach to healthcare focused on quality outcomes
- Conduct thorough patient assessments, develop individualized care plans, and monitor ongoing health needs
- Collaborate with care coordinators and other team members to ensure seamless patient care
- Utilize proprietary EMR for accurate documentation and workflow optimization
- Participate in team meetings and case conferences to discuss patient progress and optimize care strategies
- Maintain up-to-date knowledge of clinical best practices and value-based care methodologies
- Commit to a 3-6 month term with potential for ongoing engagement
Salary & Benefits
- Competitive hourly rate of $150/hr (equivalent to $312,000 annually)

Qualifications & Requirements
- Board Certification or Board Eligibility (BC/BE) in Family Medicine or Internal Medicine
- Active Virginia medical license or ability to obtain prior to start date
