Primary Care Physician (MD/DO) - Summary
As a Primary Care Physician (MD/DO), you will provide 100% outpatient care for a small panel of senior patients, focusing on preventative medicine, chronic disease management, and holistic health. You will be supported by a dedicated care team and have access to in-house consulting specialists. This position is ideal for board-certified or board-eligible physicians in Family Medicine, Internal Medicine, or Geriatric Medicine who are passionate about value-based care and improving patient outcomes.
Duties & Responsibilities
- Deliver comprehensive primary care services to a panel of up to 450 senior patients, focusing on those with multiple chronic conditions.
- See an average of 18 patients per day in a fully outpatient clinical setting.
- Collaborate with a dedicated support team including a Medical Assistant, Scheduler, and Referral Specialist.
- Utilize a value-based care approach to improve health outcomes and patient satisfaction.
- Participate in integrated care with in-house consulting specialists such as Cardiology and Podiatry.
- Offer holistic health services including acupuncture, tai chi, yoga, and nutrition counseling to patients.
- Maintain a consistent Monday through Friday schedule, with clinic hours from 7:30 AM to 5:00 PM, and last patient scheduled at 4:15 PM.
- Dedicate specific administrative time during your workday to manage documentation and care coordination.
- Ensure continuity of care and build long-term relationships with your senior patient population.
- Participate in quality improvement initiatives and professional development activities.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Board certified or board eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible for medical licensure in the state of Texas.
- Demonstrated commitment to providing high-quality, patient-centered care.
- Strong communication and collaboration skills.
- Prior experience with value-based care models is preferred but not required.
