Primary Care Physician (MD/DO) – Summary
- Deliver high-quality, value-based primary care to adult and geriatric Medicare populations
- Collaborate with a multidisciplinary care team to optimize patient outcomes
- Utilize advanced EMR systems and access to onsite lab and specialist resources
- Provide care in a modern, patient-focused clinic environment
Duties & Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide preventive, acute, and chronic disease management services
- Order and interpret diagnostic tests as needed
- Coordinate care with medical assistants, scribes, social workers, and other healthcare professionals
- Utilize EMR systems (Canopy and Greenway) for accurate documentation
- Participate in quality improvement initiatives and value-based care programs
- Engage in patient education and community health activities
- Maintain compliance with clinical protocols and regulatory standards

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted, and non-probationary medical license
- Fellowship training in Geriatrics is preferred but not required
- We are not accepting residents for this position
