Primary Care Physician (MD/DO) – Summary- Deliver high-quality, value-based primary care to adult and geriatric Medicare populations
- Collaborate with a multidisciplinary care team to optimize patient outcomes
- Utilize advanced EMR systems and access to onsite lab and specialist resources
- Provide care in a modern, patient-focused clinic environment
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide preventive, acute, and chronic disease management services
- Order and interpret diagnostic tests as needed
- Coordinate care with medical assistants, scribes, social workers, and other healthcare professionals
- Utilize EMR systems (Canopy and Greenway) for accurate documentation
- Participate in quality improvement initiatives and value-based care programs
- Engage in patient education and community health activities
- Maintain compliance with clinical protocols and regulatory standards

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted, and non-probationary medical license
- Fellowship training in Geriatrics is preferred but not required
- We are not accepting residents for this position

- Comfortable working in a collaborative, multidisciplinary team environment
- Effective communicator with strong organizational and documentation skills
- Skilled in utilizing EMR systems and open to adopting new technology
- Committed to continuous learning and participating in professional development
- Clinic supports patient transportation within a 5-mile radius
- Onsite community room offers wellness activities such as exercise, yoga, dance, and spa days
- Average patient volume is 13-16 per day, allowing for thorough, quality-focused visits
- Each clinic typically has 3-5 providers, including NPs and Primary Care Physicians
