Primary Care Physician (MD/DO) – Summary
- Deliver value-based primary care services to Medicare patient populations.
- Work collaboratively in a multidisciplinary care team environment with support from scribes, medical assistants, and social workers.
- Focus on providing high-quality care, reducing hospital admissions, and promoting preventive medicine.
- Utilize advanced in-clinic resources, including on-site labs and access to specialist consultations through platforms such as RubiconMD.
- Leverage electronic medical records (EMR) systems such as Canopy and Greenway to streamline patient documentation and care coordination.
Duties & Responsibilities
- Manage a patient panel averaging 13-16 patients per day with appointment durations tailored to patient needs (20 minutes for routine visits, 40 minutes for full diagnostic or new patient visits).
- Implement value-based healthcare principles to drive improved patient outcomes and satisfaction.
- Collaborate with other providers including nurse practitioners and physician partners within the clinic.
- Participate in and contribute to community wellness initiatives, including patient education, exercise, and social activities hosted in a dedicated community room.
- Ensure continuity of care by coordinating with care team members and leveraging in-house and remote specialist support.
- Promote preventive health measures, chronic disease management, and patient engagement strategies.
- Comply with all regulatory, legal, and ethical standards of practice.
- Utilize EMR systems efficiently for documentation, care planning, and quality reporting.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license (non-probationary status).
- Fellowship training in Geriatrics preferred but not required.
- No current residents; position requires completion of residency training.
- Commitment to providing equitable, patient-centered care in a collaborative environment.
