Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care for Medicare patients in a collaborative healthcare setting.
- Focus on preventive medicine, patient education, and reducing hospital admissions.
- Leverage advanced technology and resources, including onsite labs, EMR systems (Canopy, Greenway), and teleconsultation services.
- Engage in a patient-first environment, supported by a multidisciplinary care team.
Duties & Responsibilities
- Conduct thorough assessments, diagnosis, and treatment planning for adult and senior patients.
- Maintain patient volumes of 13-16 per day, allowing for 20- to 40-minute appointments that support comprehensive care and patient engagement.
- Collaborate closely with scribes, medical assistants, social workers, and other healthcare professionals to ensure the highest standard of care.
- Utilize onsite laboratory services and specialist consultation platforms for efficient, integrated care delivery.
- Participate in patient and community activities, including exercise, wellness events, and educational programming.
- Engage in value-based care initiatives with a focus on quality metrics and patient outcomes.
- Document all patient encounters accurately and efficiently using state-of-the-art EMR systems.
- Contribute to a positive clinic environment, supporting peers and participating in continuous process improvement.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license.
- No current residents accepted.
- Fellowship training in Geriatrics is preferred but not required.
