Primary Care Physician (PCP) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our physician-led, mission-driven organization. This full-time role is based in Delray Beach, FL, offering a rewarding opportunity for clinical professionals who are passionate about value-based medicine and making a difference in the community.
Key Responsibilities- Provide comprehensive outpatient care to a panel of senior patients, typically managing individuals with 3-5 chronic conditions.
- See approximately 18 patients per day in a clinic setting, ensuring a manageable patient load and quality interactions.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate holistic and efficient patient care.
- Employ a value-based care approach to improve patient health outcomes and satisfaction.
- Utilize administrative time included within your schedule to manage patient documentation and care coordination.
- Adhere to a consistent Monday through Friday schedule, with clinic hours from 7:30 am to 5:00 pm, and the last patient scheduled at 4:15 pm.
- Participate in ongoing professional development and clinic initiatives to improve quality of care and patient engagement.
- Refer to in-house consulting specialists, such as cardiology and podiatry, as appropriate for patient needs.
- Engage with holistic health services provided at the clinic, including acupuncture, Tai Chi, yoga, and nutrition support, to enhance patient care.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, patient-centered care.
- Strong communication, teamwork, and organizational skills.
- Experience working with senior populations and managing chronic diseases is highly valued.
