Primary Care Physician (MD/DO) - Summary
- Full-time position focused on providing primary care to a senior patient population.
- Outpatient-only setting with no night or weekend shifts required.

Duties & Responsibilities
- Conduct comprehensive patient assessments and manage ongoing care for a panel of up to 450 patients, primarily seniors with 3-5 chronic conditions.
- Provide high-quality, evidence-based medical care in an outpatient setting, ensuring continuity and coordination of care.
- Work closely with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to support each patient’s health journey.
- Utilize value-based care models to achieve optimal health outcomes for patients.
- Participate in regular administrative time built into the schedule to complete documentation and care coordination activities.
- Engage with in-house consulting specialists in fields such as Cardiology and Podiatry as needed to support holistic patient care.
- Promote holistic health services including acupuncture, tai chi, yoga, and nutrition as part of patient wellness plans.
- Attend to approximately 18 patients per day, ensuring personalized and attentive care.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care in a value-based model.
- Excellent communication and interpersonal skills, with a collaborative approach to team-based care.

- Clinic operates on a Monday through Friday schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
