Primary Care Physician (MD/DO) Job OverviewWe are seeking a dedicated and mission-driven Primary Care Physician (MD/DO) to join our organization in Delray Beach, FL. Responsibilities- Provide outpatient primary care services to a patient panel of seniors, primarily managing patients with 3-5 chronic conditions.
- Conduct patient visits, with an average of 18 patients per day, focusing on preventive and value-based care approaches.
- Collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists, to ensure coordinated and efficient patient care.
- Utilize in-house consulting specialists, such as cardiology and podiatry, to support comprehensive patient management.
- Incorporate holistic health services, including acupuncture, Tai Chi, yoga, and nutrition, into patient treatment plans as appropriate.
- Participate in administrative tasks as part of scheduled work hours, ensuring seamless clinic operations and effective patient care documentation.
- Adhere to a Monday through Friday schedule, with clinic hours from 7:30 am – 5:00 pm, and no night or weekend shifts required.
- Commit to the organization’s mission of honoring seniors with affordable, VIP care that leads to better health outcomes.
- Engage in partnership opportunities, with the potential to become a partner within 12-18 months based on performance and organizational needs.
- Medical degree (MD or DO) with successful completion of an accredited residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Demonstrated commitment to value-based care and the ability to manage a small, focused patient panel for optimal health outcomes.
- Strong interpersonal, organizational, and collaborative skills to work effectively with clinical and support teams.
- Dedication to serving the senior population with compassion and respect.
