As a Primary Care Physician (MD/DO), you will work with a dedicated support team, including your own medical assistant, scheduler, and referral specialist. You will manage a small patient panel to ensure personalized attention, focusing on seniors who often have multiple chronic conditions. Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a small panel of senior patients (maximum 450)
- Manage and coordinate care for patients with multiple chronic conditions, focusing on a value-based model
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Utilize administrative time included in your schedule to ensure quality care and effective patient management
- Work Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.)
- Participate in organizational quality improvement initiatives and embrace evidence-based medicine
- Refer patients to in-house consulting specialists, including cardiology, podiatry, and more
- Promote holistic health services such as acupuncture, Tai Chi, yoga, and nutrition support
- Ensure seamless, coordinated care including transportation assistance for patients as needed

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Family Medicine, Internal Medicine, or Geriatric Medicine
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
