Primary Care Physician (MD/DO) About the Opportunity

As a Primary Care Physician (MD/DO), you will play a vital role in transforming healthcare delivery for a population with complex medical needs. Key Responsibilities
- Provide comprehensive, 100% outpatient primary care for a senior patient population, with an average of 18 patients per day.
- Deliver care utilizing a value-based model, focusing on seniors with multiple chronic conditions (typically 3-5 per patient).
- Work with a dedicated clinical support team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated and efficient care.
- Maintain a manageable patient panel size, capped at 450 patients, to support high-quality, personalized care.
- Collaborate with in-house consulting specialists in cardiology, podiatry, and additional fields as needed to provide holistic care.
- Participate in holistic health offerings such as acupuncture, tai chi, yoga, and nutrition counseling available within the clinic setting.
- Utilize administrative time built into your schedule for charting, care coordination, and professional development.
- Uphold a Monday through Friday clinic schedule, with working hours from 7:30 a.m. to 5:00 p.m., and no required nights or weekends.
- Ensure the delivery of patient-focused, VIP care through accessible services, including complimentary transportation for patients as needed.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, senior-focused care.
- Excellent interpersonal, communication, and clinical skills with a patient-centered approach.
