The role is 100% outpatient, with no on-call, night, or weekend responsibilities, allowing you to maintain a healthy work-life balance. Administrative time is built into your schedule, and you will have your own dedicated care team, including a medical assistant, scheduler, and referral specialist. Patient transportation is provided at no charge to ensure your patients can access care easily. Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient primary care role focusing on the senior population
- Work within a mission-driven, physician-led organization

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, managing 3-5 chronic conditions per patient
- See approximately 18 patients per day with a focus on quality over quantity
- Collaborate with inhouse consulting specialists, including Cardiology and Podiatry
- Utilize holistic health resources such as acupuncture, Tai Chi, yoga, and nutrition services
- Work closely with a dedicated support team (medical assistant, scheduler, referral specialist)
- Participate in value-based care initiatives to improve patient outcomes
- Maintain accurate and timely clinical documentation
- Engage in administrative activities during built-in schedule time
- Support the organization’s mission to honor and improve care for seniors
- Follow all organizational protocols and best practices for patient care

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of an accredited residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Eligible for medical licensure
- Commitment to value-based care and senior health
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team-oriented environment
