Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a physician-led, mission-driven organization focused on delivering high-quality, value-based care. This full-time opportunity is centered on providing comprehensive primary care to seniors, particularly those with chronic health conditions, in Palm Beach County, FL. As a Primary Care Physician, you will be an integral part of a collaborative care team, committed to improving patient outcomes and honoring the organization's mission to deliver affordable VIP care to the neediest populations.
Key Responsibilities
- Provide comprehensive, patient-centered primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic diseases.
- Conduct thorough patient evaluations, develop and implement care plans, and manage ongoing treatment for chronic and acute illnesses.
- Utilize a value-based care model to achieve optimal health outcomes and patient satisfaction.
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to ensure smooth clinic operations and high-quality patient care.
- Participate in interdisciplinary case discussions with in-house consulting specialists such as cardiology and podiatry, as well as holistic health practitioners offering services like acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain accurate and timely documentation of patient encounters and care plans in accordance with organizational standards and regulatory requirements.
- Work exclusively in an outpatient setting with no night or weekend shifts, supporting a healthy work-life balance.
- Manage an average of 18 patient visits per day, with administrative time incorporated into the clinic schedule.
- Demonstrate commitment to the organization's vision by actively participating in team meetings and quality improvement initiatives.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to practicing evidence-based medicine and delivering compassionate, patient-centered care.
- Excellent communication, organizational, and teamwork skills.
- Ability to work effectively with a diverse senior patient population and multidisciplinary team.

- Full-time, outpatient-only position with a consistent Monday through Friday schedule (clinic hours: 7:30am – 5:00pm; last patient scheduled at 4:15pm).
