As a Primary Care Physician (MD/DO), you will play a pivotal role in ensuring comprehensive care for a small panel of patients. In this role, you will manage a maximum patient panel of 450, allowing you to focus on quality over quantity and foster long-lasting relationships with patients. You will provide care to seniors, most of whom have three to five chronic conditions, through a holistic, value-based care model. The practice is 100% outpatient, with support from a dedicated care team including a medical assistant, scheduler, and referral specialist. The clinic hours are Monday through Friday, and no nights or weekends are required, supporting your work/life balance. Administrative time is built into your schedule, and you will have access to in-house consulting specialists and a range of holistic health services.

Primary Care Physician (MD/DO) - Summary- Full-time outpatient opportunity delivering relationship-based, value-oriented care to seniors

- Deliver comprehensive primary care to a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct routine wellness visits, chronic disease management, and preventive care services
- Work closely with an interdisciplinary care team, including medical assistants, schedulers, and referral specialists
- Utilize value-based care principles to optimize patient outcomes and satisfaction
- Participate in care coordination activities, including referrals and follow-ups with in-house specialists (e.g., Cardiology, Podiatry)
- Incorporate holistic services such as acupuncture, tai chi, yoga, and nutrition counseling as part of patient care plans
- Maintain up-to-date and accurate documentation in accordance with organizational and regulatory requirements
- Promote patient education and engagement, empowering patients to take an active role in their health
- Attend regular team meetings and contribute to continuous process improvements

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
