Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care for a panel of senior patients, typically managing 3-5 chronic diseases per patient
- Conduct preventive care, routine check-ups, diagnosis, and chronic disease management
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist
- Coordinate care with in-house consulting specialists in cardiology, podiatry, and other disciplines
- Utilize holistic services such as acupuncture, Tai Chi, yoga, and nutrition as part of patient care plans
- Maintain a maximum panel of 450 patients for personalized attention
- Participate in value-based care initiatives to improve patient outcomes and clinic performance
- Work a Monday to Friday schedule with no nights or weekends; last patient scheduled at 4:15 PM
- Incorporate administrative time into the work schedule for documentation and care coordination
- Advocate for patient access, including supporting transportation solutions for patients in need

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to providing high-quality, patient-centered care
- Strong interpersonal skills and ability to work collaboratively in a multidisciplinary team
- Dedication to value-based care principles
- Experience managing complex, chronic conditions preferred
