As a Primary Care Physician (MD/DO), you will manage a patient panel of seniors commonly managing multiple chronic conditions. Primary Care Physician (MD/DO) - Summary- Full-time primary care role delivering outpatient services to senior patients

- Provide comprehensive primary care for a panel of senior patients, typically managing 3-5 chronic diseases per patient
- Conduct preventive care, routine check-ups, diagnosis, and chronic disease management
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist
- Coordinate care with in-house consulting specialists in cardiology, podiatry, and other disciplines
- Utilize holistic services such as acupuncture, Tai Chi, yoga, and nutrition as part of patient care plans
- Maintain a maximum panel of 450 patients for personalized attention
- Participate in value-based care initiatives to improve patient outcomes and clinic performance
- Work a Monday to Friday schedule with no nights or weekends; last patient scheduled at 4:15 PM
- Incorporate administrative time into the work schedule for documentation and care coordination
- Advocate for patient access, including supporting transportation solutions for patients in need
