As a Primary Care Physician (MD/DO), you will manage a small patient panel, allowing you to build meaningful relationships with your patients and provide comprehensive, individualized care. Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide direct patient care to a small panel of approximately 450 seniors
- Manage an average of 18 patients per day in an outpatient clinic setting
- Collaborate with a dedicated support team (MA, Scheduler, Referral Specialist)
- Utilize value-based care principles to optimize patient health outcomes
- Participate in holistic health initiatives, including consulting with in-house specialists and integrating holistic services such as acupuncture, tai chi, yoga, and nutrition counseling into patient care plans
- Maintain thorough and accurate medical records
- Participate in partnership track opportunities and contribute to a positive, purpose-driven team culture
- Follow a Monday-Friday schedule, with no nights or weekends
- Ensure administrative duties are completed efficiently within allotted scheduling
- Promote patient engagement and continuity of care, including support for transportation needs

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Eligible for medical licensure
- Experience with value-based care preferred, but not required
