Primary Care Physician (MD/DO) Job DescriptionOur physician-led, mission-driven organization is seeking a dedicated Primary Care Physician (MD/DO) to join our team in Palm Beach County, FL. As a leader in value-based care for seniors, we are committed to delivering affordable, VIP healthcare that improves patient outcomes. Our full-time Primary Care Physician (MD/DO) role offers a unique opportunity to make a meaningful impact while working within a collaborative and supportive clinical environment.
Responsibilities- Provide comprehensive primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct patient visits, averaging 18 patients per day, within a 100% outpatient setting.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated patient care.
- Utilize a value-based care model to improve health outcomes and patient satisfaction.
- Participate in holistic health initiatives, including access to in-house consulting specialists in cardiology, podiatry, and other fields, as well as services like acupuncture, tai chi, yoga, and nutrition counseling.
- Ensure administrative tasks are managed efficiently, with dedicated time included in your schedule.
- Maintain a Monday through Friday work schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. and no required nights or weekends.
- Support the organization's mission to provide transportation for patients who need assistance, ensuring accessibility to care.
- Medical degree (MD or DO) from an accredited institution.
- Residency training completed in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to value-based care principles and improving the health of the senior population.
- Strong collaboration and communication skills to work effectively within a multidisciplinary team.
