As a Primary Care Physician, you will manage a panel of senior patients, the majority of whom present with multiple chronic diseases such as heart disease, diabetes, and hypertension. Your practice will benefit from a manageable patient load, allowing you to deliver thorough, attentive care to 10-12 patients per day, with a maximum panel size of 650. Primary Care Physician (MD/DO) - Summary
- Contract opportunity for a Board-Certified Family Medicine or Internal Medicine Physician.

Duties & Responsibilities
- Deliver comprehensive outpatient primary care to a panel of senior patients (average 10-12 patients per day, panel size up to 650).
- Utilize a value-based care approach to manage chronic diseases such as heart disease, diabetes, and hypertension.
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, and referral specialists.
- Perform standard office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Participate in ongoing wellness activities and programs designed to improve patient quality of life.
- Ensure proper documentation and compliance with regulatory standards.
- Provide consultative support and referrals as needed within the care network.
- Maintain current medical knowledge to support evidence-based practice.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Board Certification in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not mandatory.
- Active, unrestricted license to practice medicine.
- Current DEA registration.
- Proficiency in routine office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
