Primary Care Physician (MD/DO) Job Overview

Key Responsibilities
- Deliver high-quality, value-based primary care services in an outpatient clinic setting, focusing on adult and senior populations with chronic and complex conditions, including heart disease, diabetes, and hypertension.
- Manage a patient panel with an average of 10-12 patients per day, ensuring continuity of care and personalized attention.
- Work collaboratively as part of a care team that includes a medical assistant, scheduler, referral specialist, and full support from clinic leadership.
- Utilize evidence-based guidelines and participate in initiatives to improve patient satisfaction, enhance HEDIS compliance, and reduce hospitalizations.
- Embrace a value-based care model that rewards outcomes such as patient satisfaction, adherence to company values, quality benchmarks, and health outcomes.
- Participate in a call schedule determined by the number of providers in the market (for example, 1:32 weeks).
- Perform office-based procedures as appropriate, including suturing, joint injections, skin biopsies, and basic wound care.
- Engage in ongoing professional development and contribute to a culture of continuous improvement and collaboration.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship in Geriatrics is a plus but not required.
- Current, unrestricted, active license to practice medicine.
- Valid DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
