In this role, you will practice within a value-based care model that emphasizes patient satisfaction, evidence-based medicine, and the reduction of avoidable hospitalizations. The position features a manageable patient load, with an average of 10-12 patients per day and a maximum panel size of 650, enabling you to develop meaningful relationships and offer ongoing, proactive care. The standard work schedule is Monday through Friday, 8am to 5pm, providing a balanced approach to work and life. Call coverage is shared among physicians in the area, ensuring flexibility and a sustainable work environment. Primary Care Physician (MD/DO) – Summary- Provide comprehensive outpatient primary care to a senior population with multiple chronic conditions
- Work as part of a value-based care team in a supportive and innovative clinical environment
- Build long-term patient-provider relationships focused on prevention and wellness
- Deliver high-quality primary care in an outpatient setting
- Manage and coordinate care for seniors with 3-5 complex diseases such as heart disease, diabetes, and hypertension
- See an average of 10-12 patients per day with a maximum panel size of 650
- Participate in a value-based care model and contribute to improved patient outcomes and satisfaction
- Collaborate with a dedicated care team including medical assistants, referral specialists, and schedulers
- Conduct office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Document clinical encounters and maintain accurate patient records
- Participate in call coverage as scheduled among providers in the area
- Adhere to company values and contribute to a positive, patient-focused culture
- Competitive base salary with total earning potential exceeding $300,000 (including bonus)

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Fellowship training in Geriatrics is a plus but not required
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and wound care
- Strong commitment to delivering value-based, patient-centered care
- Experienced in managing seniors with multiple chronic conditions
