Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time outpatient opportunity in Port Charlotte, FL. Responsibilities
- Provide comprehensive primary care services to adult and senior patients in an outpatient setting.
- Manage a patient panel with a focus on seniors with multiple chronic conditions, including heart disease, diabetes, and hypertension.
- Maintain continuity of care and build long-term relationships with patients and their families.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support, to ensure optimal patient outcomes.
- Ensure compliance with value-based care models and quality metrics, such as HEDIS standards.
- Take part in care initiatives that reduce hospital admissions and improve patient satisfaction.
- Document patient encounters and coordinate care using modern electronic health records and practice management systems.
- Participate in a shared call schedule, based on the number of providers in the market.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Promote patient engagement in wellness activities offered by the clinic, including exercise and art programs.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active medical license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficient in basic office procedures such as suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Commitment to delivering value-based, patient-centered primary care.

Employment Details
- Full-time position, Monday through Friday, 8:00 am to 5:00 pm.
- Outpatient care only—no inpatient responsibilities.
