Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive outpatient primary care to senior patients with multiple chronic conditions
- Work within a value-based care model focused on patient satisfaction and clinical outcomes
- Be part of a new, patient-centric clinic opening in May 2025

Duties & Responsibilities
- Provide primary care services to a panel of up to 650 senior patients, managing 10-12 patients per day
- Conduct patient assessments, diagnosis, and treatment planning for chronic conditions such as heart disease, diabetes, and hypertension
- Lead a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support
- Coordinate with interdisciplinary teams to ensure continuity of care and adherence to best practices
- Participate in value-based initiatives focused on reducing hospitalizations and improving HEDIS measures
- Ensure accurate documentation and maintain compliance with regulatory standards
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed
- Adhere to a Monday–Friday, 8am–5pm schedule with call duties proportionate to provider pool size

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Proficient in minor office procedures (suturing, joint injections, skin biopsies, wound care)
- Fellowship training in Geriatrics is a plus, but not required
- Commitment to high-quality patient care and professional ethical standards
