Primary Care Physician (MD/DO) – Full-Time

The Primary Care Physician (MD/DO) role is central to our patient-centric philosophy. You will work with a manageable patient panel of seniors, most with multiple chronic conditions, delivering evidence-based care in an outpatient setting. Supported by a dedicated care team—including a medical assistant, scheduler, referral specialist, and robust leadership—you will have the resources and autonomy needed to focus on what matters most: patient outcomes and satisfaction.

Primary Care Physician (MD/DO) – Summary
- Serve as a key member of the clinical team in an outpatient, value-based care setting
- Manage a panel of approximately 650 adult and senior patients, focusing on those with complex chronic conditions
- Participate in the launch and growth of new neighborhood clinics
- Contribute to a culture of respect, collaboration, and patient advocacy
Duties & Responsibilities
- Provide comprehensive primary care to adult and geriatric patients in an outpatient setting
- Develop and execute individualized care plans for patients with multiple chronic conditions, such as heart disease, diabetes, and hypertension
- Maintain a patient load of 10-12 patients per day, ensuring high-quality, thorough visits
- Lead and collaborate with your care team, including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives, focusing on patient satisfaction, HEDIS compliance, and the reduction of unnecessary hospitalizations
- Utilize technology and strong PC skills to perform office procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Participate in a rotating call schedule as determined by the number of providers in the market
- Contribute to the overall positive culture and mission of the organization

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus, but not required
- Current, unrestricted, and active medical license
- Current DEA registration
- Proficiency in minor office procedures (suturing, joint injections, skin biopsies, basic wound care)
