Primary Care Physician (MD/DO) - Summary
- Deliver best-in-class, value-based primary care to a senior patient population in Philadelphia, PA

Duties & Responsibilities
- Provide comprehensive outpatient primary care to seniors with complex medical conditions, including heart disease, diabetes, and hypertension
- Manage a panel of up to 650 patients, seeing an average of 10-12 patients per day
- Lead and collaborate with a dedicated care team (medical assistant, scheduler, referral specialist)
- Participate in a value-based care model focused on patient satisfaction, quality outcomes, and reducing hospitalizations
- Maintain accurate medical records and ensure compliance with HEDIS and other quality metrics
- Perform in-office procedures as appropriate (suturing, joint injections, skin biopsies, basic wound care)
- Engage patients and their families in care plans and health education
- Take part in a rotating call schedule, determined by the number of providers in the market
- Adhere to organizational values and standards of clinical excellence

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus but not required
- Current, unrestricted, and active medical license
- Current DEA registration
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
