Primary Care Physician (MD/DO) – Full-time Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team serving the Philadelphia, PA community. As part of our mission-driven organization, you will play a pivotal role in providing high-quality, patient-centered outpatient care focused on seniors with complex medical needs. Our model emphasizes value-based care in comfortable, accessible neighborhood clinics that foster a sense of belonging and wellness for both patients and staff.
Responsibilities
- Deliver comprehensive outpatient primary care services to a senior patient population, many with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage an average of 10-12 patients per day with a maximum panel size of 650, allowing for personalized, attentive care.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and supportive leadership, to ensure seamless coordination and continuity of care.
- Utilize value-based care principles, adhering to evidence-based clinical guidelines and quality metrics such as HEDIS compliance.
- Participate in a rotating call schedule based on the number of physicians in the market, ensuring work-life balance.
- Foster a welcoming, inclusive clinic environment that encourages patient engagement in health-promoting activities such as exercise, art, and social events.
- Perform essential office procedures, including suturing, joint injections, skin biopsies, and basic wound care, as needed.
- Maintain accurate, timely documentation and ensure compliance with all regulatory and organizational requirements.
- Support the company’s mission by embodying core values and contributing to a culture of respect, collaboration, and patient advocacy.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine in the state of Pennsylvania.
- Current DEA registration.
- Proficiency in common office-based procedures such as suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Strong commitment to delivering patient-focused, value-driven care in a collaborative team environment.
