Primary Care Physician (MD/DO) - Summary
- Deliver high-quality, personalized outpatient care to senior patients.
- Work alongside a dedicated care team including MAs, schedulers, referral specialists, and leadership support.
- Utilize a value-based care model focused on improving patient outcomes and satisfaction.
- Participate in a collaborative and innovative healthcare environment that prioritizes patient wellness and engagement.
Duties & Responsibilities
- Provide comprehensive primary care services to seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a patient panel with a maximum size of 650, averaging 10-12 patients per day to ensure quality attention and continuity.
- Perform routine office procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Collaborate with the healthcare team to coordinate care, optimize patient outcomes, and reduce unnecessary hospitalizations.
- Adhere to value-based care principles, focusing on patient satisfaction, HEDIS compliance, and reducing hospital sick days.
- Participate in call schedules as determined by the number of providers in the market, ensuring work-life balance and adequate coverage.
- Engage patients and families in care decisions, promoting a community-oriented approach to health and wellness.

Salary & Benefits
- Competitive base salary starting at $260,000, with first-year earning potential exceeding $300,000 through performance-based bonuses.
- Bonuses awarded based on patient satisfaction, adherence to company values, HEDIS compliance, and reduction in hospital sick days.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is mandatory.
- Active, unrestricted medical license to practice medicine.
- Current DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
