Primary Care Physician (MD/DO) - Summary- Full-time, outpatient primary care role in Philadelphia, PA
- Focus on value-based, patient-centered care for seniors
- Work collaboratively within a multidisciplinary care team
- Contribute to a mission-driven organization dedicated to community health and wellness
- Provide comprehensive outpatient primary care to a panel of senior patients (up to 650 individuals)
- Manage patients with multiple chronic conditions such as heart disease, diabetes, and hypertension
- Maintain an average daily patient load of 10-12 to ensure quality and continuity of care
- Collaborate with a care team including medical assistants, schedulers, and referral specialists
- Participate in a call schedule based on the number of providers in the market
- Engage in quality improvement initiatives, focusing on HEDIS compliance and reducing hospital admissions
- Perform office-based procedures including suturing, joint injections, skin biopsies, and wound care
- Promote a wellness-focused culture through patient education and community activities
- Base salary of $260,000 with first-year earning potential exceeding $300,000
- Performance-based bonuses linked to patient satisfaction, organizational values, clinical quality, and hospital utilization metrics

- Medical degree (MD or DO) required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Proficiency in outpatient clinical procedures, including suturing, joint injections, skin biopsies, and wound care
- Fellowship training in Geriatrics is a plus but not required
- Demonstrates a commitment to patient-centered, value-based care
- Strong communication and collaboration skills within multidisciplinary teams
- Passion for serving senior populations with complex medical needs
- Comfortable with office-based procedures and chronic disease management
