Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time outpatient position in Chicago, IL. Key Responsibilities
- Deliver comprehensive primary care services within a 100% outpatient setting, focusing on adult and senior patients with multiple chronic conditions.
- Manage a small patient panel (maximum of 450 patients) to ensure personalized, continuous care and enhanced patient-provider relationships.
- Conduct an average of 18 patient visits per day, supported by a dedicated clinical team including a Medical Assistant, Scheduler, and Referral Specialist.
- Utilize a value-based care model to improve patient outcomes and satisfaction, working collaboratively with in-house consulting specialists in cardiology, podiatry, and other disciplines.
- Participate in holistic health initiatives offered by the clinic, such as acupuncture, tai chi, yoga, and nutrition counseling, to support patients’ overall well-being.
- Contribute administrative expertise during scheduled administrative time, ensuring efficient practice management and quality assurance.
- Work Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.), allowing for work-life balance with no night or weekend shifts.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with 3-5 chronic diseases.
- Ensure all care delivered aligns with the organization’s mission to honor seniors with high-quality, accessible healthcare.
Qualifications
- Medical degree (MD or DO) required.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM); Geriatric Medicine fellowship training a plus.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing quality care to a diverse senior population.
- Strong interpersonal, communication, and organizational skills.
- Ability to work effectively in a collaborative, multi-disciplinary environment.
