Primary Care Physician (MD/DO) – Summary
- Deliver outpatient, value-based primary care services to a senior population.

Duties & Responsibilities
- Provide comprehensive primary care for adult and geriatric patients, with a focus on those managing multiple chronic conditions.
- Manage a small patient panel (maximum of 450 patients) to allow for personalized, high-quality care.
- See up to 18 patients per day during clinic hours (Monday through Friday, 7:30 AM to 5:00 PM; last patient scheduled at 4:15 PM).
- Coordinate with an in-house care team, including a Medical Assistant, Scheduler, and Referral Specialist.
- Utilize administrative time built into your schedule for charting, care coordination, and professional development.
- Collaborate with in-house consulting specialists in Cardiology, Podiatry, and more.
- Participate in value-based care initiatives to improve patient outcomes and population health.
- Integrate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling into patient care plans.
- Ensure compliance with clinical best practices and organizational protocols.
Salary & Benefits
- Competitive base salary in the range of $250,000 to $270,000 annually.
- Incentive bonuses available, with additional earning potential through partnership track.
- Comprehensive benefits package including health, dental, vision, and supplemental insurance for providers and their families.
- Generous paid time off: 33 days annually.
- 401(k) plan with up to 5.5% employer match.
- Continuing Medical Education (CME) allowance of $3,500 and one week for professional development.
- Signing bonus potential up to $50,000.
- Door-to-door transportation provided for patients at no charge, supporting access to care and continuity.
Qualifications & Requirements
- MD/DO degree from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Internal Medicine, Family Medicine, or Geriatrics.
- Demonstrated commitment to value-based, patient-centered primary care.
- Strong communication, interpersonal, and organizational skills.
- Eligibility for medical licensure in Illinois.
