As a Primary Care Physician (MD/DO), you will deliver outpatient care to an assigned panel of patients, primarily seniors with complex health needs. The practice operates Monday through Friday, with a predictable daytime schedule and no nights or weekends. Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to seniors with multiple chronic conditions.
- Manage a small patient panel (maximum 450 patients) to ensure personalized care.
- Conduct approximately 18 patient visits per day within clinic hours (Monday to Friday, 7:30 AM – 5:00 PM).
- Utilize administrative time included within the schedule to ensure thorough documentation and care coordination.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, and referral specialists.
- Participate in value-based care initiatives and quality improvement programs.
- Refer patients to in-house consulting specialists as needed, including cardiology and podiatry.
- Engage patients in holistic health services, such as acupuncture, tai chi, yoga, and nutrition counseling.
- Ensure compliance with all clinical protocols and regulatory requirements.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to delivering value-based, patient-centered care.
- Strong communication, organizational, and teamwork skills.
