Primary Care Physician (MD/DO) Overview
We are a physician-led, mission-driven primary care organization dedicated to providing high-quality, value-based care to seniors. Our practice is committed to transforming healthcare for vulnerable populations, ensuring every patient receives affordable and comprehensive services. We are seeking a Board Certified or Board Eligible Primary Care Physician (MD/DO) to join our team on a full-time basis in Chicago, IL. This role is ideal for compassionate medical professionals passionate about working in a collaborative environment focused on improving patient outcomes.
Responsibilities
- Provide primary care services to a panel of senior patients, focusing on preventive care and management of chronic conditions.
- Practice evidence-based medicine in an outpatient clinic setting, with no required nights or weekends.
- Manage a panel of up to 450 patients, ensuring continuity and quality of care.
- Conduct comprehensive assessments and develop individualized treatment plans tailored to each patient’s needs.
- Work collaboratively with an integrated care team, including a dedicated medical assistant, scheduler, and referral specialist.
- Utilize value-based care models to improve health outcomes and patient satisfaction.
- Engage in administrative duties as part of the work schedule, allowing for focused patient care and work-life balance.
- Participate in ongoing professional development and contribute to a culture of clinical excellence.
- Refer patients to in-house consulting specialists, including cardiology and podiatry, as needed.
- Promote holistic health services such as acupuncture, Tai Chi, yoga, and nutrition within the clinic.
- Ensure all care complies with legal, ethical, and organizational standards.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Completed residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatrics.
- Strong commitment to providing high-quality, patient-centered care to seniors with multiple chronic conditions.
- Excellent communication, organizational, and teamwork skills.
- Demonstrated ability to work collaboratively with multidisciplinary healthcare teams.
- Commitment to upholding the highest ethical and professional standards in all aspects of clinical practice.
