Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join a physician-led, mission-driven organization in Gretna, LA. This full-time position offers a rewarding opportunity to provide comprehensive, value-based care to a senior patient population. As a Primary Care Physician, you will work with a collaborative team focused on delivering high-quality, patient-centered healthcare that honors seniors and improves overall community health outcomes. Key Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, with an emphasis on seniors managing multiple chronic conditions.
- Conduct patient visits, averaging 18 patients per day, in an outpatient clinic setting. Hours are Monday through Friday, with clinic operations from 7:30 a.m. to 5:00 p.m.; the last patient is scheduled at 4:15 p.m.
- Deliver value-based care and participate in ongoing quality improvement initiatives to ensure best practices and optimize outcomes.
- Collaborate with a dedicated care and support team, including a medical assistant, scheduler, and referral specialist, to manage patient care efficiently.
- Utilize administrative time included in your schedule for patient documentation, care coordination, and professional development.
- Work with in-house consulting specialists such as cardiology and podiatry, and refer patients as appropriate for additional care needs.
- Integrate holistic health services, including acupuncture, tai chi, yoga, and nutrition consultations, into patient care plans as needed.
- Ensure compliance with all organizational policies, procedures, and applicable laws and regulations related to patient care and documentation.
- Participate in partnership opportunities, with the potential for advancement typically within 12-18 months.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligibility for medical licensure in the state of Louisiana.
- Open to candidates requiring visa sponsorship.
- Commitment to providing high-quality, compassionate care to senior patients with complex health needs.
- Strong communication, organizational, and teamwork skills.
