Primary Care Physician (MD/DO) – Summary
- Full-time opportunity for a Primary Care Physician (MD/DO) in Gretna, LA

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 450 seniors, focusing on chronic disease management and preventive care
- Conduct patient visits (average 18 per day) within clinic hours (Monday through Friday, 7:30 am – 5:00 pm)
- Work in a 100% outpatient setting with no night or weekend shifts required
- Utilize value-based care principles to optimize patient health outcomes
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist
- Participate in administrative activities with protected time built into your schedule
- Engage with in-house consulting specialists (Cardiology, Podiatry, etc.) for comprehensive patient management
- Incorporate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition support into patient care plans
- Ensure care coordination for patients requiring door-to-door transportation services

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine

- Experience or interest in managing complex chronic disease care in a senior population
