We are seeking a Primary Care Physician (MD/DO) for a full-time opportunity in Hampton, VA. Responsibilities
- Provide 100% outpatient primary care services to a senior patient population, many of whom manage multiple chronic conditions.
- Manage a small patient panel of up to 450 individuals, allowing for personalized and attentive care.
- Conduct patient visits, averaging approximately 18 patients per day, with appointments designed to support quality care and strong patient-provider relationships.
- Collaborate with a dedicated care and support team, including a Medical Assistant, Scheduler, and Referral Specialist, to streamline clinical workflows and enhance patient experiences.
- Participate in value-based care initiatives, focusing on improving patient outcomes and satisfaction.
- Utilize in-house consulting specialists, such as Cardiology and Podiatry, to coordinate comprehensive care.
- Leverage holistic health services, including acupuncture, Tai Chi, yoga, and nutrition support, to address patient wellness from a whole-person perspective.
- Maintain thorough documentation and administrative tasks within scheduled work hours, ensuring effective management of patient information.
- Participate in a partnership track program, with opportunities for advancement and leadership within the organization.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Open to candidates requiring visa sponsorship.
- Demonstrated commitment to providing high-quality, compassionate care to seniors and individuals with complex health needs.
- Strong team orientation, communication, and organizational skills.
Work Hours and Environment
- Full-time, Monday through Friday schedule, with clinic hours from 7:30 AM to 5:00 PM.
- No nights or weekends required, supporting work-life balance.
- Administrative time is integrated into the daily schedule.
- All services are provided in an outpatient clinic setting in Hampton, VA.
