In this full-time role, you will be responsible for managing a small panel of patients, enabling you to focus on quality interactions and individualized care. You will work alongside a dedicated support team, including your own medical assistant, scheduler, and referral specialist, to streamline administrative tasks and optimize patient care. Our outpatient clinic model operates exclusively on weekdays, providing a predictable schedule with no nights or weekends required, supporting a healthy work-life balance.

Primary Care Physician (MD/DO) - Summary
- Full-time Primary Care Physician role providing comprehensive outpatient care for seniors

Duties & Responsibilities
- Manage a small patient panel (maximum of 450 patients) to ensure individualized, high-quality care
- See approximately 18 patients per day in a 100% outpatient clinic setting
- Develop and implement care plans for seniors with multiple chronic conditions (typically 3-5 per patient)
- Maintain a Monday through Friday schedule with no night or weekend shifts
- Utilize dedicated administrative time to support thorough documentation and care coordination
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist
- Participate in value-based care initiatives to improve patient outcomes and satisfaction
- Coordinate with in-house consulting specialists in cardiology, podiatry, and other fields as needed
- Support holistic patient wellness through access to acupuncture, tai chi, yoga, and nutrition services

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine

- Ability to manage complex, chronic conditions in a senior patient population
