Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO) for a full-time position in Hampton, VA. This opportunity is with a physician-led organization that is transforming healthcare delivery for seniors, especially those in need. As a Primary Care Physician, you will be an integral member of a collaborative care team, serving a small panel of patients to ensure personalized and comprehensive attention in an outpatient setting.
Responsibilities
- Provide primary care services to a dedicated panel of senior patients, focusing on managing 3-5 chronic illnesses per patient.
- Deliver high-quality, value-based medical care in a 100% outpatient environment.
- Conduct patient consultations, perform routine examinations, order appropriate diagnostic tests, and develop individualized treatment plans.
- Collaborate with an in-house team including medical assistants, schedulers, referral specialists, and consulting specialists such as cardiology and podiatry.
- Leverage holistic health services including acupuncture, Tai Chi, yoga, and nutrition support as part of comprehensive patient care.
- Maintain a manageable patient panel, typically up to 450 patients, with an average of 18 patient visits per day.
- Participate in regular administrative time built into the schedule to support thorough documentation and care coordination.
- Utilize provided resources, including door-to-door transportation for patients, to remove barriers to care.
- Uphold organizational mission and values by delivering respectful and dignified care to all patients.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible to work in the United States; open to visa candidates.
- Demonstrated commitment to providing care for senior populations and managing complex chronic conditions.
- Excellent communication and interpersonal skills to engage patients and collaborate with care teams.
