Primary Care Physician (MD/DO) – Summary
- Full-time, outpatient-only primary care role focused on senior patients with chronic conditions
- Physician-led, mission-driven organization dedicated to value-based healthcare

- Small patient panel (maximum 450 patients) for personalized care

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of senior patients, primarily managing those with multiple chronic diseases (typically 3-5 per patient)
- Deliver care in a 100% outpatient setting, Monday through Friday during clinic hours (7:30 am – 5:00 pm, last patient scheduled at 4:15 pm)
- See an average of 18 patients per day, ensuring high-quality, individualized care
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to optimize patient care and clinic efficiency
- Participate in value-based care initiatives and adhere to evidence-based clinical protocols
- Allocate administrative time within your schedule for patient documentation and care coordination
- Engage in holistic health services, leveraging in-house consulting specialists (e.g., cardiology, podiatry) and wellness offerings such as acupuncture, tai chi, yoga, and nutrition counseling
- Support the organization’s mission to honor seniors and improve community health outcomes
Salary & Benefits
- Competitive base salary ranging from $250,000 – $270,000 per year
- Incentive bonus opportunities, with potential for up to $20,000 in the first year and increased earnings as a partner
- Signing bonus up to $50,000

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to value-based care and patient-centered practice
- Strong communication, collaboration, and organizational skills
