Primary Care Physician (MD/DO) – Full-time in Memphis, TN

Primary Care Physician (MD/DO) - Summary
- Full-time position as a Primary Care Physician (MD/DO)
- Provide comprehensive outpatient care for a senior patient population
- Work collaboratively within a multidisciplinary team
- Focus on value-based care and improved health outcomes
Duties & Responsibilities
- Deliver high-quality, patient-centered outpatient primary care to a panel of up to 450 seniors, many with multiple chronic conditions (typically 3–5 per patient)
- Conduct thorough assessments, develop individualized care plans, and manage ongoing treatment for a variety of acute and chronic illnesses
- Coordinate care with in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as nutrition, acupuncture, tai chi, and yoga
- Utilize value-based care principles to achieve better health outcomes and patient satisfaction
- Work a consistent weekday schedule, with no required nights or weekends
- Maintain detailed and accurate electronic health records and participate in quality improvement initiatives
- Lead and participate in regular team meetings, collaborating with medical assistants, schedulers, and referral specialists
- Ensure patient access to care by supporting transportation solutions and other patient-assistance programs
- Participate in partnership and leadership opportunities within the organization
Salary & Benefits

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Valid and unrestricted medical license
- Excellent communication, interpersonal, and organizational skills
- Commitment to continuous quality improvement and evidence-based practice
- Genuine interest in serving a diverse senior patient population
