Primary Care Physician (MD/DO) – Memphis, TN – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven, physician-led organization in Memphis, TN. As a Primary Care Physician, you will play a vital role in providing high-quality, value-based care to a senior patient population, many of whom are managing multiple chronic conditions. Our organization is focused on transforming healthcare delivery for underserved communities, honoring seniors by offering affordable, comprehensive care designed to achieve better health outcomes.
Responsibilities
- Deliver comprehensive, patient-centered primary care to a panel of up to 450 patients, focusing on seniors with complex medical needs and 3-5 chronic diseases.
- Maintain a manageable daily patient load of approximately 18 patients, allowing for more personalized and attentive care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient experiences and optimal health outcomes.
- Utilize value-based care models to improve patient health and wellness, focusing on preventive medicine and chronic disease management.
- Participate in a Monday through Friday clinic schedule, with clinic hours from 7:30 a.m. to 5:00 p.m., and the last patient scheduled at 4:15 p.m.
- Enjoy a 100% outpatient practice with no required night or weekend shifts, supporting a healthy work-life balance.
- Incorporate administrative time into your schedule for efficient documentation and care planning.
- Engage with in-house consulting specialists in cardiology, podiatry, and other disciplines to deliver holistic, coordinated care.
- Encourage patient participation in available holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
Qualifications
- MD or DO degree from an accredited medical school.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
