Primary Care Physician (MD/DO) - Summary
- Full-time permanent opportunity for a Primary Care Physician (MD/DO).
- Deliver outpatient primary care to seniors with complex health needs.

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 senior patients, focusing on preventive care and chronic disease management.
- Conduct patient assessments, develop individualized care plans, and manage ongoing treatment for patients with multiple chronic conditions.
- Work closely with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists, to coordinate patient care and ensure seamless delivery of services.
- Utilize value-based care principles to improve patient outcomes, reduce hospitalizations, and enhance the overall patient experience.
- Participate in regular administrative and care team meetings to review patient progress and optimize care strategies.
- Refer patients to in-house consulting specialists as needed, including cardiology and podiatry services.
- Integrate holistic health services such as acupuncture, tai chi, yoga, and nutrition into patient care plans when appropriate.
- Maintain accurate and timely medical records in compliance with organizational policies and regulatory requirements.
- Engage in ongoing professional development and contribute to clinical quality improvement initiatives.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to delivering value-based, patient-centered care to seniors with multiple chronic conditions.
