We focus on maintaining small patient panels to promote meaningful, long-term relationships between physicians and their patients. This allows for comprehensive care, proactive management of chronic conditions, and the delivery of exceptional service. The Primary Care Physician (MD/DO) will work closely with an integrated care team, including dedicated medical assistants, schedulers, and referral specialists, to ensure patients receive seamless, coordinated care throughout their healthcare journey.
In this full-time role, you will benefit from a Monday through Friday schedule, with no night or weekend shifts, and built-in administrative time to support a healthy work-life balance. Our practice is 100% outpatient, allowing you to focus on preventive care, chronic disease management, and developing personalized treatment plans for each patient. With a maximum patient panel of 450, our model emphasizes quality over quantity, ensuring you have the time and resources needed to make a lasting difference. Additionally, we provide door-to-door transportation for patients who require it, further removing barriers to access and enhancing the overall patient experience.

Primary Care Physician (MD/DO) – Summary- Full-time, outpatient primary care role focused on serving senior patients with chronic health conditions.

- Maintain accurate and timely medical records, ensuring compliance with all regulatory standards.
