Primary Care Physician (MD/DO) - Summary
As a Primary Care Physician (MD/DO), you will be responsible for managing a small patient panel, delivering comprehensive outpatient care to a senior population with complex health needs. Supported by a dedicated care team including a medical assistant, scheduler, and referral specialist, you will focus on providing high-touch, patient-focused care, emphasizing prevention and the management of chronic conditions.
Duties & Responsibilities
- Provide primary medical care to a maximum panel of 450 patients, focusing on seniors with multiple chronic diseases.
- Conduct thorough patient assessments, develop and implement individualized care plans, and monitor progress toward health goals.
- Work closely with a multidisciplinary care team to ensure coordinated, holistic care across specialties including cardiology, podiatry, and integrative health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Maintain a full-time, outpatient schedule with no nights or weekends, seeing approximately 18 patients per day during clinic hours from 7:30 am to 5:00 pm (last patient at 4:15 pm).
- Incorporate administrative time into your daily schedule to ensure thorough documentation and care coordination.
- Utilize a value-based care model to improve patient outcomes, focusing on prevention, chronic disease management, and patient education.
- Participate in continuous quality improvement initiatives and contribute to the organization’s mission of delivering exceptional healthcare to seniors.
- Leverage door-to-door transportation services provided to patients, promoting accessibility and continuity of care.
- Engage in partnership opportunities with a clear pathway and no buy-in, offering significant professional and financial growth potential.

Qualifications & Requirements
- Medical doctorate (MD or DO) from an accredited medical school.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
