Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Deliver primary care services to a panel of up to 450 adult and senior patients, focusing on those with multiple chronic conditions
- Conduct patient evaluations, develop treatment plans, and manage ongoing care in accordance with evidence-based guidelines
- Work Monday through Friday during clinic hours (7:30am – 5:00pm; last patient scheduled at 4:15pm)
- See an average of 18 patients per day in a 100% outpatient setting
- Utilize value-based care principles to achieve optimal health outcomes
- Collaborate with an in-house team including medical assistants, schedulers, and referral specialists
- Engage with consulting specialists in cardiology, podiatry, and other disciplines as needed
- Participate in holistic health initiatives such as acupuncture, Tai Chi, yoga, and nutrition counseling for patients
- Ensure accurate documentation and compliance with clinical protocols
- Dedicate scheduled administrative time for patient care coordination and follow-up
- Partner with the organization for ongoing quality improvement initiatives

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Active and unrestricted medical license to practice
- Commitment to value-based, patient-centered care
- Strong communication, clinical, and teamwork skills
- Desire to serve a senior population with multiple chronic health conditions
