Primary Care Physician (MD/DO) – Summary
- Full-time opportunity providing comprehensive, value-based care to senior patients
- Physician-led, mission-driven organization focused on transformative healthcare
- Manage a small patient panel (max 450) for personalized, high-quality care
- Collaborate with a dedicated team including Medical Assistants, Schedulers, and Referral Specialists
- Access to in-house consulting specialists and holistic health services
Duties & Responsibilities
- Provide outpatient primary care to a panel of senior patients, focusing on prevention, chronic disease management, and care coordination
- Conduct patient visits (approximately 18 per day), maintaining thorough documentation and follow-up as needed
- Work collaboratively with in-house specialists in cardiology, podiatry, and other disciplines to ensure comprehensive patient care
- Engage in value-based care initiatives and quality improvement activities
- Utilize available holistic health services—such as acupuncture, tai chi, yoga, and nutrition—to enhance patient wellness
- Participate in regular administrative time, included in the schedule, to manage patient care efficiently
- Partner with dedicated support staff to optimize patient experience and outcomes
- Uphold organizational mission and values in all aspects of patient care and teamwork
Salary & Benefits
- Competitive base salary with additional incentive bonus
- Potential signing bonus (up to $50,000)
- Partnership opportunity typically available within 12-18 months; no buy-in required
- Work-life balance: Monday through Friday, no nights or weekends

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency trained in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to serving complex senior patient populations
- Strong communication and interpersonal skills
