Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven organization in New Orleans, LA. As a full-time team member, you will play a pivotal role in delivering high-quality primary care to a senior patient population in a supportive, value-based care environment. Key Responsibilities
- Provide comprehensive outpatient primary care for a panel of up to 450 senior patients, typically managing 3-5 chronic conditions per patient.
- See an average of 18 patients per day, focusing on quality and continuity of care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated care.
- Utilize value-based care principles to improve patient outcomes and satisfaction.
- Participate in administrative duties with allotted time included in the work schedule.
- Maintain a Monday through Friday schedule, 7:30 am to 5:00 pm, with no nights or weekends required; last patient scheduled at 4:15 pm.
- Engage in ongoing partnership opportunities, with potential for advancement within 12-18 months based on performance and leadership.
- Leverage in-house consulting specialists (including Cardiology and Podiatry) and access to holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Support patient access by coordinating transportation services as needed.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing patient-centered, compassionate care and working collaboratively in a multidisciplinary team environment.
