Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a physician-led, mission-driven organization focused on delivering affordable, high-quality care to senior patients. Key Responsibilities
- Provide comprehensive outpatient primary care to a panel of senior patients, typically managing 3-5 chronic conditions per patient.
- Deliver care in accordance with value-based care principles, focusing on quality outcomes and patient satisfaction.
- Maintain a manageable patient panel, with a maximum of 450 patients and an average of 18 patient visits per day.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care and streamline clinic operations.
- Utilize administrative time included in your schedule to ensure thorough documentation and follow-up.
- Participate in a Monday through Friday clinic schedule, with hours from 7:30 a.m. to 5:00 p.m., ensuring no night or weekend shifts for optimal work/life balance.
- Engage in partnership opportunities with potential for advancement, with most physicians achieving partnership status within 12-18 months and no buy-in required.
- Collaborate with in-house consulting specialists in areas such as cardiology and podiatry, as well as holistic health services including acupuncture, Tai Chi, yoga, and nutrition support.
- Support patient access with door-to-door transportation services offered at no charge to those in need.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certified or board eligible in FM, IM, or Geriatric Medicine.
- Commitment to delivering patient-centered, value-based care in an outpatient setting.
- Excellent communication and teamwork skills.
