Primary Care Physician (MD/DO) Job Description
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time opportunity in New Orleans, LA. As a valued member of a physician-led, mission-driven primary care organization, you will play a pivotal role in providing high-quality, value-based healthcare to the community’s senior population. Key Responsibilities
- Deliver comprehensive outpatient primary care to a senior patient panel, focusing on those with multiple chronic conditions.
- Manage a patient panel of up to 450 individuals, with an average of 18 patient visits per day.
- Utilize value-based care practices to ensure quality outcomes and patient satisfaction.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate and optimize patient care.
- Participate in multidisciplinary team meetings and leverage in-house consulting specialists such as cardiology and podiatry to enhance patient care.
- Provide holistic health guidance, connecting patients to available services like acupuncture, tai chi, yoga, nutrition counseling, and door-to-door transportation.
- Maintain accurate and timely documentation of patient visits and care plans.
- Engage in administrative tasks during allocated time within your schedule.
- Promote a healthy work-life balance with a Monday through Friday schedule; no nights or weekends required.
Qualifications
- Medical degree (MD or DO) with completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered, and value-based care to the senior population.
- Strong interpersonal and communication skills for effective collaboration with care teams and patients.
