As a Primary Care Physician (MD/DO), you will be responsible for providing comprehensive outpatient primary care to a dedicated panel of senior patients, most of whom manage multiple chronic diseases. You will work alongside a dedicated support team, including medical assistants, schedulers, and referral specialists, to ensure each patient receives personalized, coordinated care. Primary Care Physician (MD/DO) - Summary
- Provide comprehensive primary care to a panel of senior patients in an outpatient clinic setting.
- Deliver value-based care focused on prevention, chronic disease management, and holistic wellness.

Duties & Responsibilities
- Manage a panel of up to 450 senior patients, typically with multiple chronic conditions.
- Conduct thorough patient assessments, develop individualized care plans, and monitor ongoing progress.
- Provide preventive care, chronic disease management, and acute care as needed.
- Coordinate care with in-house consulting specialists, including Cardiology and Podiatry, as well as holistic service providers.
- Utilize value-based care principles to improve health outcomes and patient satisfaction.
- Maintain accurate and timely clinical documentation and participate in quality improvement initiatives.
- Collaborate with dedicated support staff, including medical assistants, schedulers, and referral specialists, to optimize care delivery.
- Participate in regular administrative time built into the weekly schedule to ensure sustainable work-life balance.
- Contribute to a positive, inclusive, and mission-driven clinic culture.
Salary & Benefits
- Base salary starting at $250,000–$270,000 per year, with performance-based incentive bonuses.
- Signing bonus opportunities available.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care.
- Strong interpersonal and communication skills, with a collaborative approach to healthcare delivery.
- Eligibility for medical licensure in the practice jurisdiction.
