Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide high-quality primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct thorough patient assessments, develop and implement individualized care plans, and monitor outcomes
- Lead and collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives to improve patient outcomes, satisfaction, and overall health
- Utilize in-house consulting specialists (Cardiology, Podiatry, and others) and holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition) for comprehensive patient care
- Maintain accurate and timely documentation of patient encounters and care processes
- Incorporate administrative responsibilities within your workday, ensuring a balanced and efficient workflow
- Promote preventive care and healthy lifestyle choices for patients

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to providing value-based, patient-centered care to seniors
- Strong communication, collaboration, and organizational skills
- Eligibility to work in the United States; open to visa candidates
- Demonstrated passion for improving health outcomes and quality of life for seniors
