Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven clinical team in Newport News, VA. As a key member of our physician-led organization, you will play a pivotal role in delivering high-quality, value-based outpatient care primarily to a senior adult population. Our practice is committed to transforming healthcare delivery for those who need it most, providing affordable and comprehensive care that improves patient outcomes. Responsibilities
- Provide primary care services in an outpatient clinic setting to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Deliver comprehensive and coordinated care, utilizing a dedicated support team that includes a medical assistant, scheduler, and referral specialist.
- Conduct thorough patient assessments, develop treatment plans, and manage ongoing care for chronic diseases.
- Work collaboratively with in-house consulting specialists such as cardiologists and podiatrists, and refer patients as needed for specialized care.
- Maintain a daily schedule of approximately 18 patients, ensuring each receives attentive and personalized care.
- Participate in value-based care initiatives aimed at improving health outcomes and enhancing the patient experience.
- Incorporate holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling into patient care plans as appropriate.
- Allocate administrative time within the work schedule to support documentation and care coordination.
- Collaborate with an interdisciplinary team to ensure seamless patient care and support practice goals.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible for unrestricted medical licensure in the state of Virginia.
- Open to candidates requiring visa sponsorship.
- Strong commitment to value-based, patient-centered care for seniors with multiple chronic conditions.
- Excellent communication, interpersonal, and teamwork skills.
Work Schedule & Environment
- Full-time employment, Monday through Friday from 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
- No nights or weekend shifts required, ensuring a healthy work/life balance.
- All services provided in a 100% outpatient setting.
- Patient transportation provided at no cost to those who need it.
