We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven organization in Norfolk, VA. Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, primarily consisting of seniors with multiple chronic conditions.
- Conduct patient visits, averaging 18 patients per day, within a 100% outpatient setting.
- Collaborate closely with an integrated care team, including a dedicated medical assistant, scheduler, and referral specialist.
- Deliver value-based care with a focus on preventative health, chronic disease management, and holistic patient outcomes.
- Utilize in-house consulting specialists in cardiology, podiatry, and other fields to ensure comprehensive patient management.
- Coordinate with holistic health services such as acupuncture, Tai Chi, yoga, and nutrition programs to provide well-rounded patient support.
- Maintain a work schedule of Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m.; last patient scheduled at 4:15 p.m.
- Ensure administrative time is included within your daily schedule to maintain efficient patient documentation and care planning.
- Uphold the organization’s mission to honor seniors by providing affordable, high-quality care and support, including transportation services for patients who need it.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Commitment to value-based care and patient-centered practices.
- Strong communication skills and ability to work collaboratively within a multidisciplinary team.
- Open to both experienced physicians and those seeking to develop their clinical leadership skills.
