Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and patient-focused Primary Care Physician (MD/DO) to join our mission-driven, physician-led primary care organization in Norfolk, VA. This full-time opportunity is ideal for candidates who are passionate about delivering comprehensive outpatient care to a senior patient population with complex, chronic medical needs. As a Primary Care Physician, you will be part of a collaborative clinical team that is committed to transforming healthcare delivery and advancing value-based care in the community.
Responsibilities
- Provide high-quality, patient-centered primary care services to a panel of up to 450 patients, focusing primarily on seniors with multiple chronic conditions.
- Conduct outpatient clinic visits, managing an average of 18 patients per day during regular clinic hours, Monday through Friday, with no night or weekend shifts required.
- Utilize value-based care models to deliver preventative, acute, and chronic disease management in alignment with organizational best practices and clinical guidelines.
- Lead and coordinate care with a dedicated support team, including a medical assistant, scheduler, and referral specialist to optimize patient outcomes and satisfaction.
- Participate in interdisciplinary collaboration with in-house consulting specialists (e.g., cardiology, podiatry) and holistic health service providers (such as acupuncture, Tai Chi, yoga, and nutrition).
- Ensure thorough documentation and administrative tasks are completed within allocated schedule time, supporting efficient and quality-driven care delivery.
- Work closely with partners in the organization to continuously improve care processes, patient engagement, and health outcomes for the senior population.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible for unrestricted medical licensure in the state of Virginia.
- Strong passion for caring for seniors and managing patients with multiple chronic diseases.
- Excellent communication, teamwork, and patient engagement skills.
- Open to candidates requiring visa sponsorship.
