As a Primary Care Physician (MD/DO), you will benefit from a collaborative, patient-centered work environment with a manageable patient panel and robust support staff, including your own medical assistant, scheduler, and referral specialist. This full-time position offers a balanced work/life schedule with no nights or weekends, administrative time built into your day, and a maximum patient panel of 450 individuals, allowing you to provide personalized attention and continuity of care. You'll lead the clinical management of patients, focusing on quality, evidence-based practices, and establishing long-term, trusted relationships.
Our practice emphasizes value-based care, supporting our physicians with in-house consulting specialists in cardiology, podiatry, and other fields, as well as holistic health services including acupuncture, tai chi, yoga, and nutritional counseling. We also ensure comprehensive patient support by providing door-to-door transportation at no cost, ensuring access to those who need it most.

Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care to a patient panel of up to 450 individuals, primarily seniors with 3-5 chronic diseases
- Conduct thorough assessments, diagnosis, and management of acute and chronic medical conditions
- Develop, implement, and monitor individualized care plans utilizing evidence-based guidelines
- Collaborate with in-house consulting specialists (cardiology, podiatry, etc.) and holistic health service providers
- Lead and participate in interdisciplinary team meetings to coordinate patient care
- Utilize value-based care principles to optimize health outcomes
- Document all patient encounters, treatment plans, and progress notes in a timely and accurate manner
- Engage patients and families in health education and preventive care initiatives
- Participate in quality improvement and risk management activities
- Maintain compliance with all organizational policies and regulatory requirements

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Current and unrestricted medical license

- Strong communication and teamwork skills
- Willingness to work with seniors and individuals managing multiple chronic illnesses
