Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven team in Portsmouth, VA. Responsibilities
- Provide comprehensive, ongoing primary care to a patient panel primarily composed of seniors with multiple chronic conditions, utilizing a value-based care model.
- Manage a daily schedule of approximately 18 patients, ensuring high-quality, personalized, and evidence-based care.
- Participate in a Monday through Friday work schedule with clinic hours from 7:30 a.m. to 5:00 p.m.; no night or weekend shifts required.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care and streamline administrative tasks.
- Engage in administrative duties as part of your regular schedule, allowing for a balanced workload and optimal patient management.
- Utilize in-house consulting specialists, such as cardiology and podiatry, to enhance patient care and outcomes.
- Support holistic health initiatives, offering services like acupuncture, Tai Chi, yoga, and nutrition counseling to patients.
- Ensure appropriate care transitions and follow-up, particularly for patients utilizing the clinic's complimentary door-to-door transportation service.
Qualifications
- Medical Degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible for medical licensure in Virginia.
- Commitment to providing high-quality, value-based care to older adults and individuals with complex health needs.
- Strong communication, collaboration, and organizational skills.
- Open to candidates requiring visa sponsorship.
