Primary Care Physician (MD, DO) Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD, DO) to join a leading physician-led organization committed to transforming healthcare for underserved populations. This full-time position is based in Portsmouth, VA, and offers an opportunity to provide comprehensive outpatient primary care to a senior patient population with complex medical needs. Key Responsibilities
- Deliver high-quality, patient-centered primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Conduct comprehensive medical assessments, develop individualized treatment plans, and manage ongoing chronic disease care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care and ensure seamless service delivery.
- Utilize value-based care models to optimize patient outcomes, focusing on preventative care, holistic health, and chronic disease management.
- Maintain thorough and accurate documentation in accordance with organizational policies and regulatory requirements.
- Participate in a full-time, Monday through Friday schedule (clinic hours 7:30am – 5:00pm), with no nights or weekends, ensuring a strong work/life balance.
- Schedule and see an average of 18 patients per day, allowing for meaningful provider-patient interactions and tailored care.
- Engage in administrative responsibilities during protected time allotted within the workday.
- Refer patients to in-house consulting specialists, including Cardiology and Podiatry, as well as holistic health services such as Acupuncture, Tai Chi, Yoga, and Nutrition support.
- Coordinate with clinic-provided transportation services to ensure patients have access to necessary care without barriers.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Commitment to value-based, patient-centered care models, particularly for senior populations.
- Strong clinical skills in managing patients with chronic and complex medical conditions.
- Excellent communication, collaboration, and organizational abilities.
- Willingness to work in a team-based environment and actively participate in continuous quality improvement initiatives.
- Open to candidates requiring visa sponsorship.
