Primary Care Physician (MD/DO) – Summary- Core provider of primary care services to adult and senior patient population.
- Focus on value-based, comprehensive care for patients with multiple chronic conditions.
- Supported by a dedicated care team and innovative practice resources.

- Provide primary care services to a small, manageable panel of up to 450 patients.
- Deliver outpatient care exclusively—no inpatient, nights, or weekend shifts required.
- Conduct patient visits (average of 18 per day) and manage complex chronic conditions in seniors.
- Utilize value-based care models to drive improved health outcomes.
- Collaborate with in-house specialists, including cardiology and podiatry, and refer as appropriate.
- Incorporate holistic health options into patient care plans, including acupuncture, tai chi, yoga, and nutrition counseling.
- Work closely with assigned support staff (medical assistant, scheduler, referral specialist) to manage patient flow and administrative tasks.
- Participate in regular team meetings and quality improvement initiatives.
- Ensure all documentation meets compliance and quality standards.
