Primary Care Physician (MD/DO) - Summary
- Full-time outpatient role focused on primary care for senior patients

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, typically managing 3-5 chronic conditions per patient
- Conduct patient visits in a 100% outpatient clinic setting, seeing approximately 18 patients per day
- Develop, implement, and monitor value-based care plans that address the unique needs of each patient
- Collaborate with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Leverage in-house consulting specialists such as cardiologists and podiatrists as needed
- Participate in holistic health initiatives, including acupuncture, nutrition, Tai Chi, and yoga programs for patients
- Maintain accurate medical records and ensure compliance with all relevant healthcare regulations
- Utilize scheduled administrative time for charting and care coordination
- Engage in partnership opportunities within the organization, with a typical path to partnership within 12-18 months
- Ensure continuity of care by coordinating with other healthcare providers and community resources

Qualifications & Requirements
- Medical degree (MD or DO) with completion of residency training in Internal Medicine or Family Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to providing high-quality, patient-centered care
- Experience working with senior populations or those with multiple chronic conditions preferred
- Strong organizational, communication, and clinical skills
- Ability to work effectively in a team-based care model
