Primary Care Physician (MD/DO) Job Description
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven healthcare organization in Atlanta, GA. As a Primary Care Physician, you will play a key role in delivering value-based, comprehensive care to an underserved senior population. This full-time, outpatient opportunity offers a collaborative environment where you will work with a supportive clinical team focused on improving patient outcomes and enhancing the quality of care for seniors living with multiple chronic conditions.
Responsibilities
- Provide comprehensive primary care to a diverse panel of patients, with a focus on seniors managing 3-5 chronic diseases.
- Conduct patient assessments, develop individualized care plans, and manage ongoing treatment in an outpatient clinic setting.
- See an average of 18 patients per day, ensuring high-quality, patient-centered care.
- Collaborate closely with a dedicated care team including your own Medical Assistant, Scheduler, and Referral Specialist to streamline patient care and administrative processes.
- Participate in value-based care initiatives, utilizing evidence-based guidelines to optimize health outcomes and reduce unnecessary hospitalizations.
- Engage in regular communication with consulting specialists, such as Cardiology and Podiatry, to coordinate holistic care for complex cases.
- Incorporate holistic health services into patient care plans, including acupuncture, tai chi, yoga, and nutritional counseling, as available through the clinic.
- Maximize work/life balance with a Monday – Friday schedule (7:30 am – 5:00 pm), no night or weekend requirements, and administrative time included within your workday.
- Maintain accurate and thorough documentation in accordance with organizational policies and clinical standards.
- Contribute to quality improvement efforts and uphold the organization’s mission of providing affordable, VIP-level care to the community's neediest populations.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, patient-centered care with a focus on underserved or senior populations.
- Strong interpersonal and communication skills for effective collaboration within a multidisciplinary team.
- Ability to adapt to a value-based care model and embrace holistic, team-based approaches to patient management.
