As a Primary Care Physician (MD/DO), your role will focus on delivering comprehensive care to a small, manageable panel of senior patients, allowing you to build strong relationships and make a lasting impact. Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to a maximum panel of 450 senior patients, primarily managing chronic health conditions.
- Work within a mission-driven organization committed to value-based care and patient-centered outcomes.
- Leverage a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient care delivery.
- Clinic-based, outpatient-only position with a consistent Monday through Friday schedule.
Duties & Responsibilities
- Provide thorough, compassionate primary care to a panel of senior patients with multiple chronic diseases.
- Conduct patient visits (approximately 18 per day) during regular clinic hours, ensuring the highest standards of care.
- Collaborate with in-house specialists, including cardiology and podiatry, to coordinate multidisciplinary care plans.
- Utilize holistic services such as acupuncture, Tai Chi, and nutrition counseling to support patient wellness.
- Participate in value-based care delivery, focusing on preventive measures and chronic disease management.
- Engage in regular administrative time built into your schedule for documentation and care coordination.
- Work with a supportive team to ensure seamless patient referral processes and ongoing care management.

Qualifications & Requirements
- MD or DO degree from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, patient-centered care.
- Experience or interest in caring for a geriatric patient population with complex health needs is preferred.
- Excellent interpersonal, communication, and organizational skills.
