Primary Care Physician (MD/DO) Job Description
We are seeking a dedicated Primary Care Physician (MD/DO) to join our growing physician-led, mission-driven primary care organization. This is a full-time opportunity based in Atlanta, GA, providing comprehensive care to a senior patient population. As a Primary Care Physician, you will play a vital role in transforming the delivery of healthcare to some of the most vulnerable community members, focusing on value-based, patient-centered care. Our team is committed to honoring seniors by providing affordable, VIP-level care designed to improve overall health outcomes.
Responsibilities
- Deliver high-quality, comprehensive primary care services to a diverse panel of up to 450 senior patients, focusing on those with multiple chronic diseases.
- Manage an average of 18 patients per day, ensuring thorough evaluation, diagnosis, and management of acute and chronic conditions in an outpatient setting.
- Actively participate in a value-based care model, utilizing evidence-based protocols and collaborating with interdisciplinary care teams to optimize patient outcomes.
- Coordinate care with in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to streamline patient visits and administrative tasks.
- Ensure accurate and timely documentation of all patient encounters and care plans in accordance with organizational and regulatory standards.
- Participate in ongoing quality improvement initiatives and professional development activities to maintain a high standard of care.
- Utilize administrative time, included within your schedule, for patient follow-up, care coordination, and team meetings.
- Maintain a strong focus on work-life balance with a Monday through Friday schedule, no night or weekend shifts, and no inpatient or on-call responsibilities.
- Provide compassionate, culturally sensitive care to a senior patient population in the Atlanta, GA area, utilizing provided transportation services for patients in need.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care and improving health outcomes for seniors.
- Strong interpersonal and communication skills with the ability to work collaboratively in a multidisciplinary team environment.
- Ability to manage multiple chronic conditions and provide holistic care for geriatric patients.
- Eligible for medical licensure in the state of Georgia.
