Primary Care Physician (MD/DO) - Summary
- Full-time role providing comprehensive primary care to a senior patient population

Duties & Responsibilities
- Deliver high-quality, patient-centered primary care to a panel of up to 450 senior patients
- Manage and coordinate care for seniors with multiple chronic illnesses (3-5 per patient on average)
- Conduct patient visits Monday through Friday, with clinic hours from 7:30 am to 5:00 pm (last appointment at 4:15 pm)
- Maintain a daily schedule of approximately 18 patients
- Utilize value-based care models to improve patient outcomes
- Collaborate with in-house specialists in cardiology, podiatry, and other disciplines
- Incorporate holistic health services, including acupuncture, tai chi, yoga, and nutrition counseling, into patient care plans
- Work closely with a dedicated support team including a medical assistant, scheduler, and referral specialist
- Participate in administrative duties and allocate time within the work schedule for these tasks
- Foster strong relationships with patients and their families to support ongoing health and wellness
- Ensure all care provided meets the highest standards for safety, quality, and patient satisfaction

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Passion for providing high-quality primary care to seniors and managing complex chronic conditions
- Strong communication, organizational, and teamwork skills
- Commitment to value-based care and continuous improvement in clinical practice
