Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven healthcare organization in Clearwater, FL. As a Primary Care Physician, you will play a vital role in delivering value-based care to a senior patient population, focusing on providing high-quality, comprehensive healthcare in a supportive outpatient setting. Key Responsibilities
- Manage a small panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Provide comprehensive primary care services in a 100% outpatient setting, including preventive care, chronic disease management, and coordination with in-house specialists.
- Conduct patient visits Monday through Friday, with clinic hours from 7:30 am to 5:00 pm and the last appointment scheduled at 4:15 pm, ensuring a healthy work/life balance with no nights or weekends required.
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to enhance patient care delivery and administrative efficiency.
- Engage in a value-based care model, aiming to improve patient outcomes and quality of life while reducing unnecessary healthcare costs.
- Participate in multidisciplinary case reviews and work closely with in-house consulting specialists such as cardiology and podiatry, as well as holistic health service providers.
- Utilize administrative time included in your schedule for thorough documentation and care coordination.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to providing patient-centered, value-based care to a diverse senior population.
- Strong interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team.
- Demonstrated dedication to ongoing professional development and quality improvement.
