Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive primary care to a senior patient population in an outpatient setting.
- Participate in a value-based care model focused on improving patient health outcomes and satisfaction.
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists.

Duties & Responsibilities
- Manage a small patient panel (maximum of 450 patients) to ensure individualized, quality care.
- Conduct patient evaluations, develop care plans, and manage chronic conditions commonly affecting seniors (typically 3-5 chronic diseases per patient).
- See an average of 18 patients per day during regular clinic hours (Monday through Friday, 7:30 AM to 5:00 PM; last patient scheduled at 4:15 PM).
- Utilize administrative time built into your schedule for record-keeping and care coordination.
- Lead and participate in value-based healthcare initiatives to improve clinical outcomes.
- Coordinate care with in-house consulting specialists (e.g., cardiology, podiatry) and holistic health service providers (e.g., acupuncture, yoga, nutrition).
- No night or weekend shifts are required; strictly outpatient responsibilities.
- Ensure adherence to organizational policies, best practices, and regulatory requirements.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM); Geriatric Medicine training is a plus.
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Current and unrestricted medical license to practice.
- Commitment to providing high-quality, patient-centered care in an outpatient setting.
- Strong communication, organizational, and teamwork skills.
