Primary Care Physician (MD/DO) - Summary
- Full-time Primary Care Physician role (MD/DO)

Duties & Responsibilities
- Provide comprehensive outpatient primary care to a panel of up to 450 senior patients, focusing on the management of 3-5 chronic diseases per patient
- See approximately 18 patients per day in a clinic setting
- Work Monday through Friday, with clinic hours from 7:30 AM to 5:00 PM (last patient at 4:15 PM), allowing for work/life balance and no nights or weekends
- Utilize dedicated administrative time and a supportive care team, including a medical assistant, scheduler, and referral specialist, to optimize patient care and efficiency
- Participate in a value-based care model, focusing on improved health outcomes and preventive care for patients
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other disciplines as needed
- Integrate holistic health services such as acupuncture, tai chi, yoga, and nutrition into patient care plans when appropriate
- Ensure the highest standards of medical ethics, compliance, and professionalism in all patient interactions

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Strong commitment to providing patient-centered, value-based care
- Excellent communication, collaboration, and organizational skills
- Desire to work in a team-oriented, mission-driven setting serving seniors
