Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a leading, physician-led primary care organization in Clearwater, FL. This is a full-time opportunity for a practitioner passionate about delivering value-based, patient-centered care to the senior population. Key Responsibilities
- Provide high-quality, evidence-based outpatient primary care services to a senior patient population, typically managing 18 patients per day.
- Develop and implement individualized care plans, addressing 3-5 chronic conditions common among geriatric patients.
- Coordinate care with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient experiences.
- Participate in value-based care initiatives to improve patient outcomes and optimize resource utilization.
- Maintain a small patient panel (maximum 450 patients) to promote continuity of care and build long-term relationships.
- Dedicate scheduled administrative time to documentation, care coordination, and team collaboration.
- Refer patients for in-house consulting specialty services, including cardiology and podiatry, as clinically indicated.
- Promote holistic health by facilitating access to services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Uphold high standards of ethical and professional conduct, ensuring compliance with all relevant healthcare regulations.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based care and serving the senior population.
