Primary Care Physician (MD/DO) About the Role
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led and mission-driven organization in Cleveland, OH. As a full-time provider, you will be instrumental in delivering comprehensive, value-based care to a senior population with complex health needs. Our mission is to honor seniors by providing affordable, VIP-level healthcare that results in better health outcomes. This role offers an opportunity to make a meaningful impact while working in a supportive team environment focused on holistic patient care.
Key Responsibilities
- Delivering patient-centered, outpatient care to a panel of up to 450 senior patients, managing an average of 18 patients per day.
- Working collaboratively with a dedicated support team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure patients receive timely and coordinated care.
- Providing comprehensive management of patients with multiple chronic diseases, focusing on value-based outcomes and preventive care.
- Participating in a Monday through Friday clinic schedule from 7:30 AM to 5:00 PM, with no night or weekend shifts required.
- Utilizing dedicated administrative time included in your schedule to complete documentation and coordinate care effectively.
- Engaging in partnership opportunities with pathways available generally within 12-18 months.
- Collaborating with in-house consulting specialists such as Cardiology and Podiatry, as well as accessing holistic health services including Acupuncture, Tai Chi, Yoga, and Nutrition.
- Ensuring that patient transportation needs are met through provided services, enhancing access to care.
Qualifications
- Medical degree (MD or DO) and completion of an accredited Internal Medicine or Family Medicine residency program.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to value-based, patient-centered primary care with a focus on improving the health of seniors with multiple chronic conditions.
- Excellent interpersonal and communication skills, with a team-oriented and mission-driven approach.
