Primary Care Physician (MD/DO) About the Role
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time opportunity in Cleveland, OH. Key Responsibilities
- Provide comprehensive outpatient primary care services to an assigned panel of senior patients, managing both acute and chronic medical conditions.
- Maintain a manageable patient panel (maximum 450 patients) to ensure high-quality, personalized care.
- Conduct approximately 18 patient visits per day during regular clinic hours (Monday to Friday, 7:30 AM – 5:00 PM), with no required nights or weekends.
- Utilize value-based care models to optimize patient outcomes and support organizational goals.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated patient care.
- Incorporate administrative time into your schedule to handle patient documentation and care coordination efficiently.
- Participate in partnership opportunities with no buy-in requirement, with most providers achieving partner status within 12-18 months.
- Engage with in-house consulting specialists in cardiology, podiatry, and other disciplines as needed for comprehensive patient care.
- Support holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling, to promote overall wellness.
- Serve a patient population comprised primarily of seniors with multiple chronic illnesses, supporting their ongoing health and well-being.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based care and population health management.
- Strong interpersonal, communication, and organizational skills.
