Primary Care Physician (MD/DO) - Summary
- Full-time position providing value-based, outpatient primary care to a senior patient panel

Duties & Responsibilities
- Deliver primary care to a panel of up to 450 senior patients, focusing on chronic disease management and preventive health
- Conduct patient consultations, physical exams, and develop personalized care plans
- Collaborate with dedicated care teams, including medical assistants, schedulers, and referral specialists
- Utilize value-based care principles to optimize patient outcomes and satisfaction
- Coordinate care with in-house specialists, including cardiology and podiatry, as well as holistic services such as acupuncture and nutrition counseling
- Maintain accurate electronic health records and complete administrative tasks during allotted time in the daily schedule
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and innovation
- Ensure compliance with organizational policies, procedures, and all applicable regulations

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Commitment to value-based care and the mission of serving seniors with complex needs
- Strong interpersonal skills and ability to work effectively within a multidisciplinary care team
