Responsibilities
- Provide high-quality, patient-centered primary care services in an outpatient setting, focusing on adult and geriatric populations with multiple chronic conditions.
- Manage a small patient panel of up to 450 patients to ensure personalized and attentive care.
- Conduct daily patient visits, with an average of 18 patients per day, ensuring thorough assessments, diagnosis, and treatment planning.
- Collaborate with an integrated care team including a dedicated Medical Assistant (MA), Scheduler, and Referral Specialist to support patient care and administrative efficiency.
- Utilize value-based care principles to achieve optimal patient outcomes while maintaining cost-effective practices.
- Participate in care coordination and communication with in-house consulting specialists, including Cardiology and Podiatry.
- Leverage holistic health services available through the clinic, such as acupuncture, Tai Chi, yoga, and nutrition counseling, to promote overall wellness.
- Ensure all patient documentation is completed accurately and timely, with dedicated administrative time included in your schedule.
- Maintain regular communication with patients, families, and other healthcare professionals to provide education and support.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Commitment to providing compassionate, high-quality care to seniors and adults with complex health needs.
- Excellent communication, interpersonal, and teamwork skills.
- Dedication to value-based care and a patient-first approach.
Schedule & Work Hours
- Full-time, Monday through Friday schedule.
- Clinic hours from 7:30 am to 5:00 pm; last patient scheduled at 4:15 pm.
- No night or weekend shifts required, promoting work-life balance.
- 100% outpatient care with administrative time included.
