Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven primary care organization in Cleveland, OH. This full-time role offers the opportunity to make a meaningful difference in the lives of seniors by providing comprehensive and value-based outpatient care. Responsibilities
- Deliver high-quality, patient-centered medical care to a small panel of up to 450 patients, focusing on seniors with multiple chronic diseases.
- Manage an average of 18 patients per day in a 100% outpatient clinic setting.
- Work collaboratively with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure comprehensive patient support.
- Provide clinical leadership and participate in value-based care initiatives to improve patient outcomes.
- Utilize in-house consulting specialists and holistic health services, such as cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition, to support patient wellness.
- Maintain accurate and timely documentation of patient encounters.
- Ensure compliance with established clinical protocols, regulatory requirements, and quality standards.
- Participate in administrative activities, with dedicated time included in the work schedule.
- Foster strong relationships with patients and their families, delivering care with empathy and respect.
- Promote the organization’s mission and values in every interaction.
Qualifications
- MD or DO degree from an accredited medical school.
- Completion of an accredited residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Current and unrestricted medical license to practice as a Primary Care Physician in Ohio.
- Commitment to providing high-quality, value-based care to senior populations.
- Strong interpersonal, communication, and organizational skills.
- Dedication to ongoing professional development and continuous learning.
