Primary Care Physician (MD/DO) – Full-Time Opportunity

As a Primary Care Physician (MD/DO), you will have the opportunity to work with a small patient panel, allowing for more meaningful interactions and personalized care. The clinic is fully outpatient, offering a work schedule with no nights or weekends, and includes protected administrative time within your workday.

This position offers a highly competitive compensation package, including a base salary, performance-based incentive bonus, and a signing bonus. Partnership opportunities are available, typically within the first 12-18 months, with the majority of physicians advancing to partner status after one year. Primary Care Physician (MD/DO) – Summary
- Full-time outpatient primary care opportunity focused on seniors with chronic health conditions

Duties & Responsibilities
- Provide evidence-based primary care to a dedicated panel of approximately 450 patients
- Conduct comprehensive health assessments and develop individualized care plans for seniors with multiple chronic diseases
- Manage a patient schedule of approximately 18 appointments per day, Monday through Friday
- Work 100% outpatient hours with no nights or weekends required
- Collaborate closely with a multidisciplinary support team, including a medical assistant, scheduler, and referral specialist
- Participate actively in quality improvement initiatives and value-based care programs
- Utilize electronic health records for documentation and care coordination
- Engage patients and their families in shared decision-making and preventive care strategies
- Access and refer patients to in-house specialists in cardiology, podiatry, and other fields as needed
- Integrate holistic health services into patient care plans as appropriate

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certification or Board Eligibility in FM, IM, or Geriatric Medicine
- Strong commitment to value-based, patient-centered care
- Excellent communication, collaboration, and organizational skills
