Primary Care Physician (MD/DO) - Summary- Full-time position as a Primary Care Physician (MD/DO).
- Work in a physician-led, patient-focused healthcare organization.
- Serve a senior patient population with complex, chronic health needs.
- Practice value-based, outpatient primary care.
- Opportunities for clinical leadership and partnership within the organization.
- Deliver comprehensive, outpatient primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Conduct routine assessments, preventive care, chronic disease management, and health education for patients.
- Work collaboratively with an assigned care team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care and support optimal outcomes.
- Participate in value-based care initiatives designed to improve patient health and satisfaction.
- Utilize administrative time included in your schedule to complete documentation, care planning, and collaboration with the interdisciplinary team.
- Maintain regular clinic hours, Monday through Friday, with no nights or weekends required, ensuring a healthy work-life balance.
- Engage with in-house consulting specialists in fields such as cardiology and podiatry, as well as holistic health services, to provide comprehensive patient care.
- Support and promote the organization’s mission to deliver affordable, VIP care to seniors.

- Medical degree (MD or DO) and completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing value-based, patient-centered care to a diverse senior population.
- Strong interpersonal, organizational, and communication skills for effective collaboration within a multidisciplinary team.
- Dedication to clinical excellence, continuous quality improvement, and positive patient experiences.
- Experienced or early-career Primary Care Physician (MD/DO) with a passion for senior health and chronic disease management.
