Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our patient-centered team in Cleveland, OH on a full-time basis. Responsibilities
- Provide comprehensive, outpatient primary care services to a panel of adult and senior patients, focusing on those with multiple chronic conditions.
- Manage a maximum patient panel of 450 individuals, ensuring personalized care and continuity.
- Conduct an average of 18 patient visits per day, with administrative time included in your schedule to support thorough patient management.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to enhance patient experience and streamline care delivery.
- Utilize value-based care principles to improve health outcomes and patient satisfaction.
- Participate in clinic-based initiatives, including holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Coordinate with in-house consulting specialists in areas such as cardiology and podiatry as needed.
- Leverage available resources, such as complimentary patient transportation, to reduce barriers to care.
- Maintain high standards of documentation, communication, and clinical quality in all interactions.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Commitment to providing compassionate, high-quality care to adult and senior populations, with an emphasis on managing complex, chronic conditions.
Employment Details
- Full-time position, Monday through Friday, with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm).
- No night or weekend work required, supporting an excellent work/life balance.
