In this full-time position, you will work with a small patient panel, allowing for individualized attention and continuity of care. The clinical environment is designed to foster work-life balance, with predictable hours, no nights or weekends, and administrative support integrated into your schedule. You will be supported by a dedicated care team, including medical assistants, schedulers, and referral specialists, ensuring you can focus on delivering exceptional care to your patients. Primary Care Physician (MD/DO) - Summary
- Full-time primary care role with a mission-driven, physician-led organization

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, primarily seniors with 3-5 chronic conditions
- Conduct thorough patient assessments, develop individualized care plans, and provide ongoing management of chronic diseases
- Collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Utilize value-based care principles to enhance patient outcomes and experience
- Refer patients to in-house consulting specialists as needed (e.g., Cardiology, Podiatry)
- Participate in holistic health initiatives offered by the clinic, such as acupuncture, tai chi, yoga, and nutrition counseling
- Maintain accurate and timely documentation of patient care activities
- Engage in partnership and leadership opportunities within the organization
- Ensure compliance with all applicable laws, regulations, and company policies

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics
- Strong commitment to value-based care and patient-centered approaches
