Primary Care Physician (MD/DO) OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven organization in Columbus, OH. As a full-time team member, you will play a vital role in providing comprehensive, patient-centered healthcare to a predominantly senior population, many of whom manage multiple chronic conditions. This is an excellent opportunity for a Primary Care Physician (MD/DO) passionate about delivering value-based care and making a positive difference in the lives of those most in need.
Responsibilities- Provide high-quality, outpatient primary care to a panel of up to 450 patients, primarily seniors with complex medical needs including 3-5 chronic diseases per patient.
- Deliver care within a Monday through Friday clinic schedule, ensuring a work/life balance with no nights or weekends required for this full-time position in Columbus, OH.
- Maintain manageable patient volumes, seeing an average of 18 patients per day, allowing for comprehensive and personalized visits.
- Utilize administrative time built into your schedule to complete documentation and coordinate patient care effectively.
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure seamless patient experiences and outcomes.
- Engage in value-based care initiatives, focusing on prevention, chronic disease management, and improved health outcomes for the senior population.
- Participate in partnership opportunities, with most physicians achieving partnership status within 12-18 months.
- Coordinate with in-house consulting specialists in areas such as Cardiology and Podiatry, and support holistic health services, including Acupuncture, Tai Chi, Yoga, and Nutrition counseling.
- Utilize the organization’s complimentary door-to-door patient transportation service to improve access to care for those who need it.
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Commitment to providing high-quality, patient-focused care in a value-based model.
- Strong communication, organizational, and teamwork skills.
