As a Primary Care Physician (MD/DO), you will be empowered by a dedicated support team, including your own medical assistant, scheduler, and referral specialist. This enables you to focus on building meaningful relationships with your patients and delivering the highest standard of care. The role is 100% outpatient, with no night or weekend shifts, and includes built-in administrative time to ensure you can provide optimal attention to each patient.

Primary Care Physician (MD/DO) – Summary
- Provide comprehensive, value-based primary care to a panel of senior patients, focusing on prevention, chronic disease management, and wellness.
- Work collaboratively within a multidisciplinary team to deliver coordinated, holistic care.
- Enjoy a supportive practice environment with flexible scheduling, manageable patient loads, and administrative support.
Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, typically with 3-5 chronic conditions each.
- See an average of 18 patients per day in an outpatient clinical setting.
- Conduct comprehensive assessments, develop and implement individualized care plans, and monitor patient progress.
- Utilize value-based care models and coordinate with in-house consulting specialists in cardiology, podiatry, and other disciplines.
- Collaborate with medical assistants, schedulers, and referral specialists to streamline patient care and administrative processes.
- Engage patients in preventive health education and holistic wellness activities, including access to acupuncture, Tai Chi, yoga, and nutrition services.
- Participate in regular team meetings and quality improvement initiatives to enhance care delivery.
- Maintain accurate and timely clinical documentation in compliance with organizational and regulatory requirements.
Salary & Benefits
- Competitive base salary starting at $250,000, plus generous incentive and signing bonuses.
- Eligibility for partnership, typically within 12-18 months, with no buy-in required.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine or Family Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing compassionate, patient-centered care.
- Strong communication, organizational, and teamwork skills.
- Ability to engage and motivate senior patients in managing chronic health conditions and achieving wellness goals.

- Clinic hours are Monday through Friday, 7:30 a.m. to 5:00 p.m.; last patient scheduled at 4:15 p.m.
