Primary Care Physician (MD/DO) – Columbus, OH – Full-time Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a mission-driven primary care organization in Columbus, OH. Responsibilities
- Deliver comprehensive primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct outpatient visits, with a daily schedule of approximately 18 patients per day, ensuring thorough and attentive care.
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and streamline administrative tasks.
- Utilize value-based care models to improve patient health and reduce unnecessary hospitalizations or emergency visits.
- Maintain regular clinic hours, Monday through Friday from 7:30 am to 5:00 pm, with the last patient scheduled at 4:15 pm, supporting work-life balance and allowing for administrative time within your schedule.
- Participate in team meetings, case discussions, and organizational quality initiatives to continuously enhance the quality of care.
- Coordinate with in-house consulting specialists, such as cardiology and podiatry, and refer patients as needed for additional care and holistic health services.
- Engage in preventive care, chronic disease management, and patient education, empowering patients to take an active role in their health.
- Adhere to evidence-based clinical guidelines and organizational protocols to maintain high standards of care and regulatory compliance.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Residency training in Internal Medicine (IM) or Family Medicine (FM) is required.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong clinical skills and a commitment to providing exceptional, patient-centered care to the senior population.
- Excellent interpersonal and communication skills, with the ability to work collaboratively in a multidisciplinary environment.
- Demonstrated interest in value-based care models and population health management is preferred.
